Honour the memory of a loved one with an In Memoriam Donation
- Once we receive your donation, we will send the grieving family a condolence card showing that you made an In Memoriam donation.
- You will receive an official tax receipt in the mail for donations of $20 and more.
- To make direct debit donations by credit card, just complete our secure online form, phone us or use the print form.
Would you like to invite your friends and family to make a donation in the memory of a deceased loved one?
The Foundation recommends including the following message in the obituary.
"In lieu of flowers, the family would prefer memorial donations to the Montreal Heart Institute Foundation. These can be sent to 5000 Bélanger Street, Montréal, Québec H1T 1C8. Contributions can also be made by phone at 514 593-2525 or toll freeor via their Web site at www.mhifoundation.org"

By mail or fax
Just complete the form, print it out and send it to us with:
- Your credit card number, the expiration date and the type of credit card (VISA or MasterCard),
