Honour someone special by making a donation in their name

• Want to commemorate a wedding anniversary, a birthday or just recognize the wonderful care you or a family member received at the Montreal Heart Institute? Then make a donation in honour of that special someone!
• Once we receive your donation, we will send the person a specially addressed card showing that you made a donation in their honour.
• You will receive an official tax receipt in the mail for donations of $20 and more.
• To make direct debit donations by credit card, just complete our secure online form, phone us or use the print form.

Four ways to make a donation:

  Online
Please complete this online form and click on SUBMIT to send it via our secure server. All the information is encoded so you can safety enter your credit card number.

  By phone
Phone us at 514 593-2525 or toll free 1 877 518-2525 and have your credit card number and the expiration date on hand.

  By mail or fax
Just complete the form, print it out and send it to us with:
- Your credit card number, the expiration date and the type of credit card (VISA or MasterCard),
  OR
- A cheque or money order payable to The Montreal Heart Institute Foundation.
